Progressive multifocal leukoencephalopathy, known as PML, is a rare and serious brain infection that occurs in people with weakened immune systems. This study focuses on patients who are immunocompromised but do not have HIV and are not taking certain medications for chronic inflammatory diseases. The study aims to evaluate the effectiveness of a medication called pembrolizumab in treating PML. Pembrolizumab is given as an injection into a vein and is being tested to see if it can help reduce the amount of a virus called JCV in the fluid around the brain and spinal cord.
The main goal of the study is to see if pembrolizumab can lower the JCV viral load in the cerebrospinal fluid of patients with PML. The study will monitor patients over a period of time to check for changes in the virus levels. Participants will receive pembrolizumab and will have regular tests to measure the virus in their cerebrospinal fluid. These tests will be done at the start of the study and then monthly for three months. The study will also look at other health outcomes, such as changes in neurological status and any side effects from the treatment.
Throughout the study, various health assessments will be conducted to track the progress of the disease and the patient's overall health. These assessments include neurological tests and scales that measure the level of disability or dependence in daily activities. The study will also record any adverse events or complications that may occur. The results will help determine if pembrolizumab is a safe and effective treatment option for patients with PML who have a compromised immune system.
